Nearly 20% of Medicare Advantage plans lose star revenue for 2023

Just half of Medicare Advantage plans with prescription drug coverage received ratings that will earn them bonuses for the 2023 plan year, down from nearly 70% in the current year, the Centers for Medicare and Medicaid Services revealed Thursday.


Two hundred and sixty Medicare Advantage plans with pharmacy benefits received at least four stars on CMS' five-point scale, down 19.2% from the prior year. That’s despite a 7.6% increase in the number of policies reviewed, which reached 507 for the coming plan year.


CMS awards 5% quality bonuses to Medicare Advantage insurers that earn at least four stars, which grants them the maximum amount the government will pay. Health insurance companies depend on these bonuses to offer generous supplemental benefits and zero-premium plans, which help differentiate their offerings in a competitive marketplace.


More than 60% of UnitedHealthcare's 74 plans received at least four stars, down from 85.1%, according to a Modern Healthcare analysis of CMS data. UnitedHealth does not believe this analysis is accurate based on its initial assessment, a spokesperson wrote in an email. UnitedHealth, which issued the most plans that received a star rating for 2023, is the nation’s largest Medicare Advantage carrier with 6.9 million members. Just 1.8% of Centene's plans received a rating of at least four stars this year, down from 32.6%, according to the Modern Healthcare analysis. Centene lost the most stars among large insurers. The insurer offered 55 policies, which is the second-most that received a rating in the program this year. The company did not immediately respond to an interview request.


For some companies, it is not the number of plans that received a high rating that's significant, but the size of the plans' membership.


Nearly 38% of Aetna's policies received at least four stars, down from 55.6%. But ratings for the CVS Health subsidiary's largest plan, the Aetna National PPO, fell to 3.5 stars, dropping the product below the four-star bonus threshold. More than 1.9 million individuals are enrolled in the Aetna National PPO, which represents 59% of the company’s Medicare Advantage membership.


Aetna attributed the decline to poor scores in the Consumer Assessment of Healthcare Providers and Systems survey (CAHPS). The company identified the loss of stars revenue as a headwind for 2024 earnings and plans to mitigate some of the downside through “ongoing contract diversification efforts,” according to a Securities and Exchange Commission filing submitted Wednesday.


"This year ratings declined industry wide," an Aetna spokesperson wrote in an email. "CMS increased reliance on member surveys to one third of the total star score. With our largest national PPO contract’s results based on less than 1,000 member survey responses out of nearly 2 million members—less than one-tenth of 1%—we don’t believe they accurately reflect the total experience of our Medicare Advantage members."


Health insurance trade association AHIP attributed the industry-wide declines to the ongoing COVID-19 pandemic, as well as to CMS modifying how it calculated ratings this year. CMS made several changes, such as doubling the weight that CAHPS scores have on star ratings.


CMS also assessed Medicare Advantage plans using its pre-pandemic standards. The agency relaxed how it calculated Medicare Advantage star ratings for 2021 and 2022 as part of COVID-19 relief, which led a record number of insurers to garner high scores and a corresponding increase in federal spending on bonuses.


“We look forward to engaging with CMS to address methodological and other issues to ensure that there are accurate, reliable indicators of plan performance,” AHIP President and CEO Matt Eyles said in a statement.


The majority of Medicare Advantage members remain enrolled in high-performing plans, Eyles said. Approximately 72% of all Medicare Advantage members with Part D benefits are covered by plans that scored at least four stars, according to CMS...

I have not seen too many plans that deserve a 4 or 5 star rating. Pharmacists have the ability to facilitate the process for patients complaints. This will bring down star ratings and maybe improve plan satisfaction. At this moment patients do not know how to complain.
